Our most comprehensive package, the AgeWell Plan addresses the big questions that come with aging and future planning. "Do I need a home health aide?" "Does it make sense to move my bedroom to the ground floor?" "Would a stair glide or walk-in shower make life easier and safer?" "Is it time to transition to a wheelchair?" "Would working with a therapist help me feel stronger and keep me from falling?" We help you understand life's challenges in the context of your abilities, and the supports and barriers in your home environment, to find the answers. With an eye toward the current level of function and a time when limitations arise, this plan is grounded in our Treatment Triangle: occupational therapy; equipment; and home modification.

Our HomeWell Plan is designed for individuals or pairs seeking expertise when planning home modifications aimed at aging in place, without the need of an evaluation of health and function. This package is suited to those who do not have any functional challenges presently, are not managing chronic conditions, and require consultation with an Aging In Place Advisor prior to starting a remodeling project. The HomeWell Plan report can be provided to the remodeling and/or architecture firm with whom the client is working. Alternatively, Integrative Aging In Place can facilitate work with preferred affiliates who specialize in aging-in-place design.

Most of us could drop a few pounds, get stronger and move a little better. But the bottom line is this: you're great just as you are. Modifying your environment can help you feel it. We conduct a thorough environmental evaluation - starting with the AgeWell or HomeWell Plan - and provide detailed recommendations for home modifications. Integrative Aging In Place works closely with our preferred affiliates in the fields of design and build and architecture. We can also consult with the builder of your choice.

Equipment can be low-tech items including grab bars in the bathroom, dressing aids, or a raised seat with handles making it easier to get on and off the toilet. It can be larger items such as a stair glide or wheelchair lift (even elevators!). Integrative Aging In Place evaluates the need and provides equipment or facilitates installation through a preferred vendor. We then provide training to ensure safe use and maximize function.
Homes take wear and tear. Time goes by and what we first placed on our list of to-dos moves to our list of things we learn to live with. Cost
Occupational therapy serves those individuals who have experienced a health event within the past year such as a fall, hospitalization, or change in health or functional status. It applies to some cases in which the individual is managing one or more chronic conditions. Service is covered by Medicare and supplemental insurance. Initiation of service requires a referral from your primary care provider or appropriate specialist. Integrative Aging In Place can help obtain the script from your doctor to get the process started.
